Position: Behavioral Health Case Manager II
Location: Remote within OR, WA, CA, ID or UT
Shift: 8am-5pm PST
The Behavioral Health Case Manager II is responsible for
performing case management telephonically for members with
behavioral health and substance abuse disorder needs. Primary
duties may include but are not limited to:
- Work telephonically with members identified as having more
intensive mental health and/or addiction challenges, in addition to
account specific requests.
- Uses appropriate clinical interviewing tools, case
conceptualization and clinical judgment skills to assess member
needs and barriers to successful recovery.
- Develops member-specific care plans to address recovery
objectives and goals, barriers and social determinates of health
needs as identified during assessment.
- Monitors and evaluates effectiveness of care plan and modifies
plan as needed, using motivational interviewing to help member
achieve recovery goals and objectives.
- Provide psychoeducation about diagnosis, treatment plan
compliance, care options and self-management strategies
- Supports member access to appropriate quality and cost
- Coordinates with internal and external resources to meet
identified needs of the members and collaborates with
