Family Medicine physician to join the growing team of providers at
the expanding Hawks Prairie Family Medicine clinic. Take advantage

Support - Team based care with RN co-visits and integrated clinical
services (PharmD, BHI, RD) - Patient-centered medical home - Shared

is $304,209 per year - The compensation amounts listed may include
productivity or quality/service incentives, call pay, extra shift
