summary: This Social Worker position involves providing case
coordination and comprehensive social work services such as
psychosocial assessment, treatment planning, crisis intervention,
and discharge planning within outpatient community settings. The
role requires a Master's Degree in Social Work and relevant
Washington State social worker licensure. The position supports
patient-focused care at Pacific Medical Centers, emphasizing

Responsible for providing case coordination and a full range of
social work services including psychosocial assessment, treatment
planning, utilization management, therapeutic interventions,
discharge planning, crisis intervention, referral and resource, and

accredited school. Coursework/Training Specific training in the
provision of outpatient community services. Coursework/Training in
using efficient, focused treatment approaches to patient problems.
