Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 1 full year of Adult Home Health Experience or 1 full year of Hospice Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Must have recent specialty experience. Must be proficient in OASIS and HomeCare HomeBase (HCHB). Must have reliable transportation, a current driver’s license, and agency-required liability insurance. Description: Provides coordination and direction for home nursing care according to physician orders. Follows agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Conducts home visits and attends periodic office meetings. Traveler should be willing to travel up to 75 miles from the home clinic. The average number of patients per day is between 5-10. Each type of visit and activity holds a different point value. Documentations and travel contribute to points for productivity assessment. Types of cases include Start of Care, Wound Care, and Follow-up Visits. All documentation must be completed within 24 hours of the patient appointment. Delayed and incomplete documentation that impacts billing and follow-up patient care will result in a cancellation. It is expected that the clinician will accept all patients assigned, regardless of distance from previous appointments. This position does not guarantee hours, and the clinician must meet weekly productivity points.
