Job Description:

Provides case management for assigned patient populations, utilizing clinical expertise, communication, and problem-solving skills to optimize outcomes. Promotes cost-effective care by coordinating resources, minimizing fragmentation, and facilitating smooth patient/family transitions across the healthcare continuum. Conducts needs assessments upon admission and regularly, facilitating referrals and linking patients to health, wellness, and post-acute services. Encourages interdisciplinary collaboration to progress care and discharge plans, ensuring appropriate length of stay and resource management. Ensures compliance with regulations, documents accurately in the medical record, and manages referrals for medications, DME, hospice, and home health. Supports care transitions, advocates for patients and families, and participates in performance improvement initiatives. Collaborates with utilization review nurses, performs face-to-face assessments, and communicates effectively across teams to meet patient needs.
